Documents Required for DV
- Online Application Form — printout / hard copy.
- Matriculation Certificate — supporting date of birth and marks.
- ITI Certificate — issued by NCVT in the relevant trade (original + one set of attested photocopies).
- Medical Fitness Certificate — in the prescribed Annexure-A format, issued by a Government-authorised medical officer (not below Assistant Surgeon rank).
- Two identical passport-size photographs and a valid identity proof (Aadhaar / Voter ID / Driving Licence).
- Category / disability / Ex-Serviceman certificates and Employment Exchange registration certificate, wherever applicable.
- Fee payment proof, wherever applicable. No TA/DA is payable for attending document verification.
